If you are a veteran or are working with a veteran to apply for a phone, select "Individual". This can be completed by anyone, as long as the form contains the veteran's information. 

If you are an organization who wants to learn more about the program, select "Organization".

IMPORTANT NOTE: If you request AFNET pays for the service, YOU MUST SHOW PROOF OF LOW-INCOME. 

This option is limited and only for low-income/homeless veterans. You will NOT be sent a phone without it. 

After selection, wait 1-2 seconds for the form to appear. DO NOT HIT SUBMIT BEFORE FILLING OUT THE REST OF THE FORM.

Your subscription is tied to an email. You can use another person's email temporarily if needed. 

Whenever you are ready to have your own account with a new email,

**All phones will have access to internet/email. The flip phone/candy bar phone are limited on apps to download. 
**The smartphone is NOT an iPhone/Samsung. This program is designated for individuals who cannot afford a phone with his/her current income. 
**You can view all of our plans/billing cycles on our site, and you can change your plan in your account. There are options with higher GB of data, and the option to pay monthly, quarterly, semi-annually or annually. 
IF YOU ARE ASKING AFNET TO PAY, YOU MUST SHOW PROOF OF LOW INCOME. This can be tax documentation, government welfare programs, pay stubs or a referral an organization/representative. 

THIS PROGRAM IS HIGHLY LIMITED. PLEASE DO NOT ASK AFNET TO PAY UNLESS COMPLETELY NECESSARY. 
In order to port, YOU MUST HAVE PREVIOUS BILLS PAID OFF. 

We will need these things from your previous carrier. List in box above. 
1. Address on your account
2. Account number
3. PIN number
4. Phone number you are porting
Upload the proof for the above. (Ex: DD214 or Veteran's License for veterans, Military ID for Active Duty, etc.)
