Have a picture of your ID (DD214 or veteran's license for veterans, military ID for active duty, etc.) available to upload before you start. All required documents must be submitted for a quick verification.

After selection, wait 1-2 seconds for the form to appear. DO NOT HIT SUBMIT BEFORE FILLING OUT THE REST OF THE FORM. 
**All phones will have access to internet/email. The flip phone/candy bar phone are limited on apps to download. 
**The smartphone is NOT an iPhone/Samsung. This program is designated for individuals who cannot afford a phone with his/her current income. 
**You can view all of our plans/billing cycles on our site, and you can change your plan in your account. There are options with higher GB of data, and the option to pay monthly, quarterly, semi-annually or annually. 
We will need these things from your previous carrier. List in box above.

3. PIN number
4. Phone number you are porting
Upload the proof for the above. (Ex: DD214 or Veteran's License for veterans, Military ID for Active Duty, etc.)
